Vision 2020 Australia has responded to the Primary Health Care Advisory Group’s (PHCAG) options paper, Better Outcomes for People with Chronic and Complex Health Conditions through Primary Health Care, contending that system change at local, regional and national levels is required to embed approaches that support well-coordinated, multidisciplinary chronic disease management.
The submission discusses effective and appropriate patient care through enrolment in a health care home, effective and efficient patient pathways, innovative ideas on how technology could contribute to improving patient outcomes, measuring and reporting patient health outcomes and suitable payment mechanisms which would best support chronic and complex care through primary health.
